To Our Patients

If you are a new patient or an existing patient that has had a recent change of insurance, is it important to confirm that we are an in-network provider with your plan.  While we are in-network with most insurances, there are a few plans that we are considered out-of-network.  For these out-of-network plan, you may incur higher costs or you may not have coverage at all.  It is important to confirm this with your insurance plan.  To determine in-network participation, you can call the member services number on your card and confirm if the provider you see is in network.  It is important to give them the specific name of the doctor (Dr. Suzette Chin, Dr. Timothy Lin, or Dr. Melissa Rhodes) and not the practice name.

The following is a list of plans that we are out-of-network:

Please leave a message and someone will return your call. The following are steps we are still implementing to ensure the highest level of safety and service for our patients. We appreciate your support and taking these steps with us.

    • Please call ahead to schedule your office or telemedicine appointment.
    • Only the patient and one caregiver are allowed in the office.
    • All patients and caregivers will have their temperatures taken and a series of questions will be asked before entering the office. Once screened, patients are usually placed directly into an exam room so as to minimize exposure to other individuals.
    • Patients and caregivers are encouraged to wear a mask during appointments. These will not be provided by staff.
    • Patients and caregivers have the option of waiting in the car ideally or lobby for their appointment and have been spaced 6’ apart for safety.
    • Hand sanitizers are placed throughout the office.
    • All workstations, counters, lobby, bathrooms and treatment areas will be sanitized throughout the day.

Patients will still receive a reminder call or text for appointments. There is a fee for less than 24 hour cancellation notice as well as patients not showing up for their scheduled appointments.

We support our patients having access to their health information through the Patient Portal. Beyond the Patient Portal, we can enable certain mobile apps that meet requisite technical specification and security requirements to provide you access to your health information in a safe and secure way. Currently no mobile apps have been configured to connect to our system. Our portal will be updated if any additional apps become available for you to use to access your health information.
